The ‘Support and home care programme for patients with social needs at the final stage of life’ will attend to 300 people throughout 2022.

300 people and their families will benefit this year in 2022 from the Support and home care programme for patients with social needs at the final stage of life. The Ramon Molinas Foundation and the Paliaclinic Foundation will be collaborating for the tenth consecutive year to care for and support socioeconomically vulnerable patients at their final stage of life, and their families, so that they are able to cope with the process with dignity and in their own homes.

The collaboration between the two entities began a year ago. In 2012, the Paliaclinic Foundation launched the Support and home care programme for patients with social needs at the final stage of life. The initiative benefited 50 people and their families and was developed as part of the Muntanya Primary Care System in Barcelona incorporating the neighbourhoods of Horta, Sant Andreu and Nou Barris. This year, as well as increasing the number of beneficiaries of the programme by sixfold, the programme will be rolled out across the whole city.

The initiative, which plans to provide 5,000 care services throughout 2022, will provide the necessary support to patients and their families so that they are able to face the final stage of life and their own death with dignity, with as little suffering as possible, in the place of their choice and with the people they love. The Support and home care programme for patients with social needs at the final stage of life provides assistive devices, home care, art therapy, music therapy and companionship to people with terminal illness, all resources that are provided by different areas of the public health care system of the Catalan Health Institute (ICS) for a given period.

The programme provides help quickly, aiming to empower families and strengthen their skills as caregivers, reduce the emotional suffering of patients and their families, and expand social support networks.
